By submitting this form, I hereby release Kappa Alpha Theta Foundation and its agents and employees from all claims, demands, and liabilities whatsoever in connection with this information. I grant Kappa Alpha Theta Foundation and any applicable Fraternity or Foundation affiliates the right to use, reuse, and edit the information I have provided herein, including the use of my name, information, photographs, and details I have provided in any communications, publications, and announcements for nonprofit purposes. I grant Kappa Alpha Theta Foundation and any applicable Kappa Alpha Theta Fraternity or Foundation affiliates the right to use, reuse, and edit the information I have provided herein, in my application, and in any other correspondence, including, but not limited to, use of my name, photographs, and details of my award(s). I grant permission for information related to me to be used in any electronic, digital, and printed communications, publications, and announcements for nonprofit purposes and all Fraternity and Foundation business, including, but not limited to, Foundation and Fraternity social media posts, verbal and published award announcements to my chapter, members of Kappa Alpha Theta Fraternity, institutions of higher education, and/or the general public. I waive the right to inspect or approve any publication of the information described above.
